We reserve the right to decline or refuse service to any individual from participating on any program. Your signature on this form certifies that your statements below are true and that you understand and comply with the health and fitness requirements of the program.

Describe relevant medical history including but not limited to diabetes, respiratory issues, high or low blood pressure, heart conditions, hearing or vision impairment, cancer, seizures, etc.

Terms and Conditions Reservation Policy - Deposit is due with application at time of reservation. -
Northwest departures: A non-refundable deposit of $300.00 per person secures your reservation.
Payment Policy - All balances are due 120 days prior to departure date. - Payments can be made by credit card with a surcharge of 3.0%, or bank transfer.
- Participants whose balances are not received by the 120 day deadline will be cancelled and all fees forfeited.
- Late payments are subject to a $50.00 late payment fee. - A number of our trips have specific reservation and payment terms. See trip details.
Refund Policy -
Full refunds, less reservation deposit, will be provided 121 days prior to departure date unless otherwise specified. - No refunds will be provided 120 days prior to departure date. - All cancellation requests must be emailed to Northwest Alpine Guides.
As departures require a minimum number of participants, Northwest Alpine Guides must adhere to a stringent refund policy. The policy is intended to protect participants who have otherwise committed time and resources to the departure. Cancellation will not result in a refund. As we offer a personalized service, we will attempt to accommodate changes and cancellations when necessary, waiving certain fees when feasible. Northwest Alpine Guides reserves the right to waive any fees.
Conditions & Participant Responsibility Mountain Gurus, LLC doing business as (DBA) Northwest Alpine Guides its owners, sponsors, agents, employees, officers, directors, associates, affiliated companies, and subcontractors (herein after collectively referred to as "MG"). Mountain Gurus screens applicants to ensure participants are placed on climbs, courses, treks and/or mountaineering expeditions that fit their experience, fitness level and expectations. MG reserves the right to accept, decline or refuse service to any individual from participating on any climb, course, trek, or expedition at any time. Participants are responsible for their own well-being. Participants are responsible for their health and physical conditioning, pre-departure planning, visa fees, domestic and international airfare, cost of evacuation, any incurred expense due to early departure, cost of evacuation, delay or trip cancellation, airport taxes, excess baggage charges, lost or damaged baggage, additional porter or transport charges, preparation of proper personal gear and clothing, personal hygiene, items of a personal nature or any charges incurred as a result of delays beyond the control of Mountain Gurus. Participant also agrees to follow the appropriate Leave No Trace practices. Participants are prohibited from using illegal drugs and alcohol while on MG climbs and courses. If MG staff, or a Photographer employed by MG, take photographs or video of Participants, Participants give consent for those photographs or video to be used by MG and/or Photographer for marketing and any other purposes and in any media, including in MG publications and on the MG website. MG reserves the right to change the price of, cancel or withdraw any climb, course, trek or expedition for any reason whatsoever prior to departure. After departure, MG reserves the right to alter or omit any part of the itinerary, to substitute hotels or leaders, to change any means of conveyance without notice and without allowance of refund, with liability for increased costs (if any) incurred by the climb, course, trek or expedition member at any time. MG hereby give notice that they act only as agents for hotels, transportation companies, land operators and suppliers of travel services. MG assumes no responsibility or liability in connection with the operation or service of any aircraft, motor vehicle, other conveyance, lodge, or hotel which may be used wholly, or in part, for services to Mountain Gurus and its clients. MG, its operators, airlines, and agents will not be responsible for any act, error, omission, nor for any injury, loss accident, delay, inconvenience, irregularity, or damage which may be occasioned by any cause whatsoever. This includes acts of nature, hazards of traveling in politically unstable areas, civil disturbance, government restrictions, war, terrorism, or failure of any means of conveyance to adhere to published schedule. In consideration of the services furnished me, and to be furnished me as a member of this climb, course, trek and/or mountaineering expedition, I hereby release MG and all the members of the climb, course, trek and/or mountaineering expedition from any and all damages, injuries, losses, or any cause of action which may result in me, my legal representatives or others purporting to exercise statutory or other rights arising out of, or in connection with this expedition. And I hereby assume each and every damage incident to my participation, and agree to indemnify and hold harmless MG and all members of the climb, course, trek and/or expedition against any sums which they, or any of them may be subject to pay in consequence of any claim or demand by or through me, or resulting from my being a member of this climb, course, trek and/or mountaineering expedition. MG cannot guarantee that you will reach the summit. Weather, route conditions, your own abilities, or the abilities of other climbers may create circumstances that make an ascent unsafe, and you or your entire party may have to turn around without reaching the summit. Failure to reach the summit due to a person’s own lack of fitness or to any of the events associated with mountaineering (such as weather, route, avalanche hazard, team dynamics, etc.), are not MG responsibility and will not result in refund or reschedule.

VISITOR’S ACKNOWLEDGEMENT OF RISKS
In consideration of the services of Mountain Gurus (DBA) Northwest Alpine Guides their officers, agents, employees, and stockholders, and all other persons or entities associated with those businesses (hereinafter collectively referred to as “MG”) I agree as follows:
Although MG has taken reasonable steps to provide me with appropriate equipment and skilled guides so I can enjoy an activity for which I may not be skilled, MG has informed me this activity is not without risk. Certain risks are inherent in each activity and cannot be eliminated without destroying the unique character of the activity. These inherent risks are some of the same elements that contribute to the unique character of this activity and can be the cause of loss or damage to my equipment, or accidental injury, illness, or in extreme cases, permanent trauma or death. MG does not want to frighten me or reduce my enthusiasm for this activity, but believes it is important for me to know in advance what to expect and to be informed of the inherent risks. The following describes some, but not all, of those risks:
The following describes some, but not all, of those risks: the hazards of walking on uneven terrain and slips and falls; being struck by rockfall, icefall or other objects dislodged or thrown from above; the use of climbing ropes and equipment; the forces of nature, including lightning, weather changes and avalanche; the risks of falling off the rock, mountain or into a crevasse; the risks of exposure to insect bites; the risk of altitude and cold including hypothermia, frostbite, acute mountain sickness, cerebral and pulmonary edema; my own physical condition, and the physical exertion associated with this activity.
I am aware that mountaineering, hiking, climbing, and camping entails risks of injury or death to any participant. I understand the description of these inherent risks is not complete and that other unknown or unanticipated inherent risks may result in injury or death. I agree to assume and accept full responsibility for the inherent risks identified herein and those inherent risks not specifically identified. My participation in this activity is purely voluntary, no one is forcing me to participate, and I elect to participate in spite of and with full knowledge of the inherent risks.
I acknowledge that engaging in this activity may require a degree of skill and knowledge different than other activities and that I have responsibilities as a participant. I acknowledge that the staff of MG has been available to more fully explain to me the nature and physical demands of this activity and the inherent risks, hazards, and dangers associated with this activity.
I certify that I am fully capable of participating in this activity. Therefore, I assume and accept full responsibility for myself, including all minor children in my care, custody, and control, for bodily injury, death or loss of personal property and expenses as a result of those inherent risks and dangers identified herein and those inherent risks and dangers not specifically identified, and as a result of my negligence in participating in this activity.
I have carefully read, clearly understood and accepted the terms and conditions stated herein and acknowledge that this agreement shall be effective and binding upon myself, my heirs, assigns, personal representative and estate and for all members of my family, including minor children.

Travel Insurance Northwest Alpine Guides strongly recommends you purchase a travel protection plan (Travel Insurance) to protect your investment in travel, personal assets and health during your trip. This will eliminate much financial risk and decrease your need to worry about things beyond your control. We provide a recommended policy offering competitive insurance rates with a company that has proven reliable, fair and prompt in responding to claims. It is important to obtain insurance immediately upon signing up for your trip in order for pre-existing conditions to be covered. We do not require you to purchase travel insurance, however we know many travelers who have benefited by doing so. The insurance carrier assumes risks that would otherwise be yours. Northwest Alpine Guides is not an insurance carrier and does not insure you against such things as theft, baggage loss, cancellation, emergency evacuation, medical treatment, flight delay or cancellation, airline or service provider bankruptcy, jury duty, personal or family sickness leading to cancellation, U.S. Department of State Travel Warnings about terrorism issued for your destination, or other risks as described in the insurance policy and in Northwest Alpine Guides terms and conditions.
By signing below, I acknowledge and I have been advised that travel insurance is recommended and available at additional cost. I understand Northwest Alpine Guides will have no responsibility to compensate me for losses that may have been covered by the recommended travel protection plan.
